The authors suggest that SGLT2 inhibitors alleviate systemic inflammation, enhance myocardial energy metabolism, and decrease interstitial fibrosis to cover the main pathophysiological processes in HFpEF, including diastolic dysfunction and systemic vascular stiffness. Other symptoms such as exertional dyspnea and exercise intolerance were also lower in patients who were put on empagliflozin.
