Accepted file formats for submission include .docx, .pdf, and .ppt files. It is important to note that submissions via cloud-based platforms such as Google Docs or OneDrive are not allowed. Additionally, your submission must adhere closely to the provided rubric, which details the specific criteria against which your work will be evaluated.

Requirement A: Clinical Practice Experience Record

What must be included in the Clinical Practice Experience (CPE) Record submission?

The CPE Record should be submitted as a standalone, fully completed, and signed document, separate from the e-portfolio. This record must contain the following:

• Full name (first and last)

• Course instructor’s name
• Completion dates for all documented activities
• A typed signature confirming the completion of all activities for both Phase 1 and Phase 2

7. Team-Building Strategies	A list of seven different strategies used to strengthen teamwork.
8. Interprofessional Stakeholders	A list of five key stakeholders from different professional disciplines.
9. Meeting Agenda	Four agenda items prepared for the initial advocacy action team meeting.
10. Phase 2 GoReact Screenshots	One screenshot of your video and two of your feedback on peers’ videos during Phase 2.
11. Phase 2 Video Reflection	A written reflection of your experiences during Phase 2.
