The standard 5A, Coordination of Care, aims at the part the psychiatric-mental health nurse practitioner (PMHNP) plays in coordination and integration of healthcare to meet the physical and psychological needs of their clients. In this competency, the competencies include teamwork/facilitating referrals, facilitating patient-centred care, and service continuity in different settings. The PMHNP enables the holistic care plan in complex conditions patients and alleviates care fragmentation through accurate coordination (American Nurses Association [ANA], 2022).

The issue of the PMHNP role in identifying, developing, and providing educational interventions based on the level of understanding and desire to learn is involved in Standard 5B, Health Teaching, Health Literacy, and Health Promotion. This competence also entails promotion of preventive care, wellness and self-management. It is also expected that PMHNPs deliver information in a manner that is culturally competent, clear, and evidence-based to empower their patients and their families to make the correct decisions in managing their health due to its significant impact on treatment efficacy and compliance.

Addressing the Questions

The main issue with Standard 5A is associated with the fact that the PMHNPs complete the intake assessment, the admission physical, and prescribing medications. The implications of the scope of practice are great as well due to the integration of physical and mental assessment that PMHNPs are also trained to perform. The doubled capability elevates the holistic demand of care, besides necessitating the delicate management of prescribing behavior. Work coordination facilitates the interaction with primary care providers and specialists and alleviates risks and enhances patient safety. The scenario where the clients are insisting on the drugs or treatment, which is informed by the information they have learned on the Internet, is one of the most common obstacles in Standard 5B. In this scenario, the PMHNP is compelled to be empathetic and respectful and overcome an understanding of the patient who would wish to know and mystify the patients. As an example, the patient may demand a certain antidepressant, about which he or she has read on social media, without being aware of its contraindications and side effects. I would, as a PMHNP, be in a position to verify their effort to conduct research, therewith provide evidence-based facts and explaining the risks, benefits, and other alternatives. It leads to trust and facilitation in the process of sharing decisions, improvement of health literacy, and assurance that treatment is not influenced by online trends as opposed to clinical practices.

Integration of Evidence

The existing sources emphasize the role of PMHNPs in decreasing the gaps between physical and mental health services. According to Collins et al. Example of Application

An example can be a patient with comorbid diabetes and depression, who is 45 years old. The PMHNP will also involve the endocrinologist in monitoring the metabolic side effects in order to provide a complete physical and psychiatric examination and prescribing of antidepressants to the patient. In the meantime, the PMHNP focuses on the health education related to lifestyle and the way physical exercise enhances mood and glucose levels. In other scenarios, the PMHNP will respect the evidence-based practice through negotiation of the little evidence, the possible interaction, and safer alternatives, whereby the patient questions about an herbal supplement that he or she has read online, and he or she will initiate using evidence-based practice and engage the patient.

Overall, there are two standards, which are 5A and 5B, that indicate the importance of the PMHNP to the coordination of care across the different disciplines and improvement of patient health literacy by teaching and promoting it. They do not just limit to clinical skills because they also require empathy, collaboration, and good communication. The PMHNPs advance evidence-based and holistic psychiatric care through the facilitation of security in prescribing, addressing misinformation, and knowledge empowerment among the patients.
