Based on what you observed of the volunteer in your recorded psychiatric interview that you submitted in Week 4, please respond to the following questions using the assignment guidelines and rubric.

1) Mental Status Examination: Watch the psychiatric interview documentation that you completed in the week 4 and note the psychiatric state of your volunteer. Record your observations of the following components of the mental status exam (MSE) using the following terms. 

Appearance

The client is a 68-year-old male, well-groomed, and dressed in clean and age-appropriate clothes. He looks a little bit overweight (BMI 33.8) but well-groomed. He sits in an upright posture and has the right eye contact during the interview.

Behavior and attitude

He is willing, relaxed, and amicable at the communication. No agitation, psychomotor disturbances, and unusual movements were present. He is open-minded and easily communicates.

Speech

Normal rate, volume and rhythm of speech. He is articulate and has no threats of monotony, slurring of articulation, pressured speech, or poverty of speech.

Affect and mood

Affect will be congruent to his mood as he reports it to be one of content, but lonely at times. Affect is complete, firm and fitting to the situation of the conversation.

Thought process

The way of thinking is goal oriented, logical and linear. The client is not disorganized in his ideas, there is no circumstantiality, tangentiality, flight of ideas, or thought blocking.

Thought content

Denies or denies any delusions, hallucinations, obsessions, phobias. The client cannot report suicidal or homicidal thought. He complains at times of his weight and financial situation, but these are not delusional.

2) Cognitive Examination: Elaborate on the age-specific exam procedures that might be employed in order to establish the cognitive condition of your volunteer. There should be the following components:

Age of volunteer

68 years old

Level of awareness or wakefulness

The client is alert and fully oriented to person, place, and time (oriented ×3).

Attention and concentration

Shows normal attention and concentration. Capable of serial 7s subtraction, as well as being able to spell the word WORLD backwards without any trouble.

Memory

 Immediate recall: accurately repeats 3 objects immediately after presentation.

 Recent memory: recalls events of the day, including what he ate for breakfast.

 Remote memory: accurately recalls past employment and important life events.

Judgment

Displays good judgment. In case of a hypothetical situation (What would you do if you smelled smoke in your home?), He takes appropriate actions to respond to the issue, i.e. informing others and calling an ambulance.

Insight

Demonstrates reasonable understanding of his health and lifestyle. Understands that his obesity and poor income could affect his health and is willing to change to healthy habits.

3) Reflection:

The open and active questions I used during psychiatric interview were to allow the client to share everything about his experiences and lifestyle problems including eating tendencies, physical activities, and social wellness. This would contribute to the full understanding of his habits and health risk and is consistent with the strategies that encourage self-reliance and independence in older adults (Vilar-Compte et al., 2021).

Strengths:

  1. Established a rapport and trust to ensure that the client feels free to communicate about personal health challenges of interest and lifestyle habits.
  2. Successful application of empathic emotional communication was applied during the exploration of daily life and problems with diet and exercise that reflects the best practices of engaging aging adults in society (Seron-Arbeloa et al., 2022).

Areas for Development:

  1. Improve the probing questions so that more information is obtained about the psychosocial variables which may affect adherence to healthy habits, and they may include economic factors (Huangfu et al., 2024).
  2. The longer pauses in the conversation will give the client a chance to elaborate on his thoughts about health and the choice of activities (National Institute on Aging, 2024).
