SW: [Flemish smiling] Good afternoon, Maria. It is nice to see you here. What about you and the boys, eh?
Maria: We’re alright. Tired, maybe. But we’re here.
SW: You see I would prefer you here even when you are tired. That shows the extent to which you are committed to serving your family. Continuing, I should mention just briefly that what you want to accomplish is what we are going to discuss today, and what we can do together to accomplish what you want. Does that sound okay?
Maria: Yes, that would help, I suppose.
SW: Great. And just like every time, we just discuss it, and it is all confidential unless there is a safety consideration. This is something that you are in charge of. I do not come here to decide on your behalf, to teach or advise you.
Discovering Maria View and Continuities of Previous Sessions.
SW: In our last session, you had said that there are three serious problems, which include the behavior of Jose in school, the health problems of Ryan, and your stress in financial and parenting. This morning, I should like to start off by asking you what impressed you most of all since our last meeting? What’s been on your mind?
Maria: Mostly Jose. The school called again. He pushed a boy during recess. He must have some counseling, they say… and maybe medicine again. I don’t know. I’m scared of all that.
SW: [Reflecting], I hear that you are overwhelmed, and you do not know what to do regarding the recommendations of the school.
Maria: Yes. I just would not want them to think that I am a bad mother.
SW: [Reframing gently] Maria, what I can see is that you are so worried about giving protection to your son and knowing what is really the best thing in his life. Not that you are a bad mother; here you are a considerate one.
Maria: [Soft sigh] I hope so.

SW: Oh, I would wish to know a bit more about what goes on before Jose gets angry. What then comes before pushing or fighting? Have you noticed a pattern?
Maria: Other children make fun of him, according to him. Or that they don’t let him play. And then he gets mad fast.
SW: And when he gets mad, what do the other kids do? How does the school respond?
Maria: They inform him that he must relax. However, he claims that nobody hears him. And then it is something larger.
SW: [In short] It seems that the sequence is as follows: a provocation (teasing), Jose not listening, and then being very responsive, which only exacerbates the situation.
Maria: Yes, that’s exactly it.
SW: The actualization of the said pattern will allow us to plan our actions on how to make him feel confident and teach him how to cope with big feelings before they burst.

SW: Maria, what do you want to see about Jose in the next few months?
Maria: I would want to see that he should be more peaceful… happier… have friends.
SW: Mind-blowing, high-purpose goals. And how peaceful in the world would be the ordinary? As an illustration, what else would you think he would do other than react with anger?
Maria: Could we speak more? Come on, tell me what has happened, and not yell and go off.
SW: Improved communication, more emotional, less physical aggression?

Transition to Goal Setting
SW: Under your instructions, I would like to contribute to the formulation of a treatment plan with you. This will involve goals-the big changes you wish to see, and objectives, which are the minor steps we can undertake to accomplish the goals. You get to take the lead here. Let’s start with Jose.
Goal 1: Improve the Emotional and Behavioral Control of Jose.
SW: Do you believe that the primary goal you have with Jose is to help him rid himself of anger and healthily release his feelings?
Maria: That is the most important thing.
SW: Great. That is even broken down into smaller goals. For example:
Two coping strategies, including deep breathing or words, will be taught to Jose to cope with frustration. In order to come up with emotional skills and confidence, Jose will be counseled on a weekly basis. Check-ins between Maria and Jose (10 minutes daily) will be introduced, during which they will talk about feelings. How do those sound?
Maria: Counseling… maybe. I’m nervous about it. But the rest sounds okay.
SW: You want to assist him, yet you have issues related to trusting professionals.
Maria: Yes.
SW: It is not necessary to lose control and attempt counseling. It means adding support. It is you who will be involved in every single step.
Maria: Okay. Maybe we can try.
Goal 2: Address the Health Needs of Ryan.
SW: Now let’s talk about Ryan. What is the most important to you about his health?
Maria: His breathing. Some nights it gets scary.
SW: Understandably so. The appropriate goal you might propose to Ryan is the improved management of his heart condition and asthma. We can set objectives like:
Always ensure that you check up and visit the doctor.
Send you to a cheap doctor of the heart in children.
Help you receive financial aid programs for medication and treatment.

Goal 3: Increased Stress Management and Stabilization of Maria.

Maria: That is reasonable, I think.
SW: Potential goals would be:

Which of them appears to you the most important?
Maria: Job programs and, probably, the support group. I do not literally converse with the other adults.
